Using Custom Forms with Products and Registrations
Edit a product > Custom Forms tab — Attach forms to products for data collection at checkout
Once you have created a custom form, you can attach it to products, seasons, or registration processes to collect information from members at the right time.
Attaching Forms to Products
Custom forms can be linked to any product in your store. When a customer purchases the product, they are prompted to fill out the form during checkout.
- Navigate to Admin > Shop and edit the product
- Find the Custom Form section
- Select the form you want to attach
- Choose when the form appears (during checkout or after purchase)
- Click Save
Common use cases:
- Registration products: Collect player info, emergency contacts, medical details
- Team products: Collect jersey size, number preference, position
- Membership products: Collect additional member profile data
- Event products: Collect dietary preferences, accessibility needs
Attaching Forms to Seasons
You can also attach forms at the season level, so anyone registering for the season completes the form:
- Navigate to Admin > Seasons and edit the season
- Find the Registration Form section
- Select your custom form
- Click Save
Viewing Submitted Form Data
After members submit forms, you can view their responses:
- Navigate to Admin > Forms > Submissions
- Select the form you want to review
- Browse submissions, filter by date or member
- Click on individual submissions to see full details
You can also export form submissions for use in spreadsheets or other tools.
Editing Form Responses
Administrators can edit submitted form responses if corrections are needed. Open the submission, make changes, and save.
Form Data and Player Profiles
Some form fields can be configured to update player profile data automatically. For example, a jersey size field on a registration form can populate the player profile so it is available for team admins and organizers.
Multiple Forms
Products and seasons can have multiple forms attached. Members will be prompted to complete each form in sequence. Use this for separating different types of information (e.g. one form for player details, another for a liability waiver).
Best Practices
- Keep forms short and focused on essential information
- Use separate forms for different purposes (registration vs waiver vs preferences)
- Review submissions regularly to ensure data quality
- Export data periodically for backup
- Update forms between seasons to reflect any changes in requirements
- Communicate clearly to members why the information is needed
