Creating Custom Forms
Admin > Customize > Custom Forms — Shows form list with fields, responses, Edit/Preview buttons
Custom forms let you collect additional information from members during registration, checkout, or other processes. Build forms with various field types to gather exactly the data you need.
What Are Custom Forms?
Custom forms are configurable data collection tools you can attach to products, seasons, or registrations. They let you ask members for information beyond the standard fields, such as emergency contacts, medical information, jersey preferences, or skill levels.
Creating a Form
- Navigate to Admin > Forms
- Click Create New Form
- Give your form a name (e.g. Player Registration Form, Medical Waiver)
- Add a description to explain the purpose
- Click Save
Adding Fields
After creating a form, add fields to collect the data you need:
- Click Add Field
- Select the field type:
- Text: Short single-line text input
- Textarea: Multi-line text for longer responses
- Number: Numeric input
- Email: Email address with validation
- Phone: Phone number input
- Date: Date picker
- Dropdown: Select from predefined options
- Radio Buttons: Choose one from multiple visible options
- Checkboxes: Select multiple options
- File Upload: Allow members to upload documents or images
- Enter the Field Label (what the member sees)
- Set whether the field is Required or optional
- Add Placeholder text or Help text if needed
- For dropdown/radio/checkbox fields, add the available options
- Click Save Field
Field Ordering
Fields appear in the order you set. Use the Ranking value to control display order, or drag and drop fields to reorder them.
Conditional Logic
Some field types support basic conditional display. For example, you might show an additional question only if the member selects a specific option from a dropdown.
Form Preview
Before attaching a form to a product or registration, preview it to make sure it looks correct and all fields are properly configured.
Best Practices
- Only ask for information you actually need
- Mark truly essential fields as required, keep others optional
- Use appropriate field types (e.g. date picker for dates, not a text field)
- Add help text for fields that might be confusing
- Test your form before attaching it to a live product
- Group related fields together logically
